Doctor's notes
What has changed / what I noticed
BP has been a little higher in the evenings (around 140/86) than in the mornings. Getting up at night more often in the last two weeks: twice on most nights. No dizziness or chest pain.
Medicine changes
Telmisartan 40 mg: continue, but take it at night instead of in the morning. No other changes.
Next steps and tests before the next review
Keep checking BP morning and evening for the next 4 weeks (the cuff sends it here). Blood test before the next review: HbA1c, lipid profile, kidney function. If BP stays above 150/95 for 3 days, call the clinic.
